    Proton pump inhibitors are associated with increased risk of development of chronic kidney disease

    Background Acute interstitial nephritis secondary to proton pump inhibitors (PPIs) frequently goes undiagnosed due to its subacute clinical presentation, which may later present as chronic kidney disease (CKD). We investigated the association of PPI use with the development of CKD and death. Methods Two separate retrospective case–control study designs were employed with a prospective logistic regression analysis of data to evaluate the association of development of CKD and death with PPI use. The population included 99,269 patients who were seen in primary care VISN2 clinics from 4/2001 until 4/2008. For evaluation of the CKD outcome, 22,807 with preexisting CKD at the first observation in Veterans Affairs Health Care Upstate New York (VISN2) network data system were excluded. Data obtained included use of PPI (Yes/No), demographics, laboratory data, pre-PPI comorbidity variables. Results A total of 19,311/76,462 patients developed CKD. Of those who developed CKD 24.4 % were on PPI. Patients receiving PPI were less likely to have vascular disease, COPD, cancer and diabetes. Of the total of 99,269 patients analyzed for mortality outcome, 11,758 died. A prospective logistic analysis of case–control data showed higher odds for development of CKD (OR 1.10 95 % CI 1.05–1.16) and mortality (OR 1.76, 95 % CI 1.67–1.84) among patients taking PPIs versus those not on PPIs. Conclusions Use of proton pump inhibitors is associated with increased risk of development of CKD and death. With the large number of patients being treated with proton pump inhibitors, healthcare providers need to be better educated about the potential side effects of these medications

    In this article we investigate the Casimir effect in the presence of a medium by quantizing the Electromagnetic (EM) field in the presence of a magnetodielectric medium by using the path integral formalism. For a given medium with definite electric and magnetic susceptibilities, explicit expressions for the Casimir force are obtained which are in agree with the original Casimir force between two conducting parallel plates immersed in the quantum electromagnetic vacuum.Comment: 8 pages, 1 figur

    Local food supplementation and psychosocial stimulation improve linear growth and cognitive development among Indonesian infants aged 6 to 9 months

    Background and Objectives: To evaluate the effect of culturally-relevant food supplementation and psychoso-cial stimulation on infant growth and development. Methods and Study Design: A community-based random-ized controlled trial was conducted in 40 clusters from 5 selected villages in Tanah Datar District of West Su-matera, Indonesia. We assessed 355 infants aged 6 to 9 months at the beginning of the study. The infants were di-vided into 4 groups: 1) Food Supplementation (FS); 2) Psychosocial Stimulation (PS); 3) Food Supplementation and Psychosocial Stimulation (FS+PS); and 4) Control Group (CG). The formula food supplement was comprised of a variety of local food sources (local MP-ASI) and adjusted for the local habits. The quality of psychosocial stimulation was assessed with the Infant HOME inventory method. Progress at 6 months was assessed by anthro-pometry and the Bayley scores of cognition, language and motor function. Results: There were improvements in linear growth, cognitive and motor development of children in the FS (p<0.05) and the FS+PS (p<0.01) groups compared to the CG. After six months of intervention, mean length increased to 6.862.08 cm and 6.662.41 cm for FS and FS+PS respectively (p<0.05). With the combination of food supplementation and psychosocial stimu-lation (FS+PS), cognitive development increased to 21.412.2 points (effect size 0.56) (p<0.01) and motor devel-opment increased to 20.718.4 points (effect size 0.50) (p<0.001). Conclusion: Combined intervention with local food supplementation and psychosocial stimulation improved infant growth, cognitive and motor development.     Generalized Forward-Backward Splitting

    This paper introduces the generalized forward-backward splitting algorithm for minimizing convex functions of the form F+∑i=1nGiF + \sum_{i=1}^n G_i, where FF has a Lipschitz-continuous gradient and the GiG_i's are simple in the sense that their Moreau proximity operators are easy to compute. While the forward-backward algorithm cannot deal with more than n=1n = 1 non-smooth function, our method generalizes it to the case of arbitrary nn. Our method makes an explicit use of the regularity of FF in the forward step, and the proximity operators of the GiG_i's are applied in parallel in the backward step. This allows the generalized forward backward to efficiently address an important class of convex problems. We prove its convergence in infinite dimension, and its robustness to errors on the computation of the proximity operators and of the gradient of FF. Examples on inverse problems in imaging demonstrate the advantage of the proposed methods in comparison to other splitting algorithms.Comment: 24 pages, 4 figure

    A QoE Model for Mulsemedia TV in a Smart Home Environment

    The provision to the users of realistic media contents is one of the main goals of future media services. The sense of reality perceived by the user can be enhanced by adding various sensorial effects to the conventional audio-visual content, through the stimulation of the five senses stimulation (sight, hearing, touch, smell and taste), the so-called multi-sensorial media (mulsemedia). To deliver the additional effects within a smart home (SH) environment, custom devices (e.g., air conditioning, lights) providing opportune smart features, are preferred to ad-hoc devices, often deployed in a specific context such as for example in gaming consoles. In the present study, a prototype for a mulsemedia TV application, implemented in a real smart home scenario, allowed the authors to assess the user's Quality of Experience (QoE) through test measurement campaign. The impact of specific sensory effects (i.e., light, airflow, vibration) on the user experience regarding the enhancement of sense of reality, annoyance, and intensity of the effects was investigated through subjective assessment. The need for multi sensorial QoE models is an important challenge for future research in this field, considering the time and cost of subjective quality assessments. Therefore, based on the subjective assessment results, this paper instantiates and validates a parametric QoE model for multi-sensorial TV in a SH scenario which indicates the relationship between the quality of audiovisual contents and user-perceived QoE for sensory effects applications

    Coping with physical and psychological symptoms: a qualitative study of advanced lung cancer patients and their family caregivers

    PURPOSE: Advanced lung cancer patients have high rates of multiple physical and psychological symptoms, and many of their family caregivers experience significant distress. However, little is known about strategies that these patients and their family caregivers employ to cope with physical and psychological symptoms. This study aimed to identify strategies for coping with various physical and psychological symptoms among advanced, symptomatic lung cancer patients and their primary family caregivers. METHODS: Patients identified their primary family caregiver. Individual semi-structured qualitative interviews were conducted with 21 advanced, symptomatic lung cancer patients and primary family caregivers. Thematic analysis of interview data was framed by stress and coping theory. RESULTS: Patients and caregivers reported maintaining a normal routine and turning to family and friends for support with symptom management, which often varied in its effectiveness. Whereas support from health-care professionals and complementary and alternative medicine were viewed favorably, reactions to Internet and in-person support groups were mixed due to the tragic nature of participants' stories. Several cognitive coping strategies were frequently reported (i.e., changing expectations, maintaining positivity, and avoiding illness-related thoughts) as well as religious coping strategies. CONCLUSIONS: Results suggest that advanced lung cancer patients and caregivers may be more receptive to cognitive and religious approaches to symptom management and less receptive to peer support. Interventions should address the perceived effectiveness of support from family and friends

    Development of a Symptom Management Intervention: Qualitative Feedback from Advanced Lung Cancer Patients and their Family Caregivers

    Background: Little is known about cancer patient and family caregiver preferences for the content and format of nonpharmacologic interventions. Revising interventions based on patient and caregiver feedback before implementation may improve intervention feasibility and acceptability, especially in the context of advanced-stage cancer. Objectives: The aim of the study was to obtain feedback from patients with advanced-stage, symptomatic lung cancer and their family caregivers on the content and format of a nonpharmacologic symptom management intervention under development. The intervention blended evidence-based cognitive-behavioral and emotion-focused strategies to reduce physical and psychological symptoms. Methods: Semistructured qualitative interviews were conducted with 21 patients with advanced-stage, symptomatic lung cancer and caregivers. Participants reviewed handouts regarding intervention components and provided feedback. Results: Patients and caregivers desired intervention components that addressed the patient's high symptom burden such as education regarding treatment adverse effects and the provision of various coping tools. Offering interventions with a brief or flexible length and delivering them via telephone were other suggestions for enhancing intervention acceptability. Participants also preferred an equal focus on patient and caregiver concerns and a more positive intervention framework. Conclusions: Intervention preferences of patients with advanced-stage lung cancer and caregivers underscore the severity of the disease and treatment process and the need to adapt interventions to patients with high symptom burden. These preferences may be incorporated into future intervention trials to improve participant recruitment and retention. Implications for Practice: Nurses can modify interventions to meet the needs of patients with advanced-stage, symptomatic lung cancer and caregivers. For example, flexibility regarding intervention content and length may accommodate those with significant symptoms

    From Things to Services: A Social IoT Approach for Tourist Service Management

    In the context of Internet of Things (IoT), the cooperation and synergy between varied and disparate communicating objects is strained by trustworthiness, confidentiality and interoperability concerns. These restrictions can limit the development of IoT-based applications especially considering the emergent boost in the number of communicating objects and their growing itinerant nature in a collective service context. A new perspective arises with the paradigm of Social Internet of Things (SIoT), that relies on the implementation of semi-independent communicating objects with cooperation assessed by social relations and social feed-back. In this article, we present the development and expansion of the IoT concept towards SIoT in the context of the interactions between tourist services as communicating objects. As a proof-of-concept we propose a composition of services as virtualized social objects and the interaction between them, by taking into consideration the balance, trustworthiness, cooperation and synergy of services. Furthermore we present a solution to integrate also accessibility in SIoT services. The presented concept is presented using a demonstrator build for tourist services

    Detection of Hepatitis E Virus Antibodies in Dogs in the United Kingdom.

    Hepatitis E virus (HEV) genotypes 3 and 4 are zoonotic pathogens, with pigs predominantly implicated in disease transmission. The rapid rise in human cases in developed countries over the past decade indicates a change in epidemiology of HEV, and it has been suggested that additional animal species may be involved in transmission of infection. Multiple studies have identified contact with dogs as a risk factor for HEV infection in industrialised nations, and a low seroprevalence to HEV has previously been reported in dogs in low-income countries. In this study we aimed to evaluate the possibility that dogs are susceptible to HEV, and determine the frequency with which this occurs. Serum samples from UK dogs with and without hepatitis were screened for HEV-specific antibodies, and canine liver and stool samples were analysed by qPCR for the presence of HEV RNA. We describe evidence to show HEV infection occurs at low levels in dogs in the UK, but the strain of origin is undetermined.
